The condition was misdiagnosed as her having a heart attack; had she been diagnosed and treated with antibiotics in a timely manner she probably would have recovered. She suffered multiple-organ failure and he watched her pass away. Having served as a pilot in the Israel Air Force and as a professional Air Safety Investigator, Douglas turned his attention to improving patient safety.
