As far as ibogaine cardiotoxicity, the key is the prolongation of the QT interval. This is occasionally congenital, but generally due to medications (like methadone and azithromycin). Hypomagnesemia and hypokalemia are also important risk factors for cardiac problems, and often present in the malnourished. While you might not need to completely medicalize ibogaine, the mortality rate is quoted at 1/300; by doing screening of ECG and bloodwork, that might be low enough to begin widespread use.”
