As part of the 2024 Developmental Disabilities Conference, Dr. Megumi Okumura, Medical Director of the newly established UCSF Health Pediatric to Adult Transitions Program, describes a roadmap for health care transition, the process of moving from a child/family centered to an adult/patient centered model of health care; and introduces tools from a recently updated Got Transition, a collaborative effort by the American Academy of Pediatrics, the American Academy of Family Physicians, and the American College of Physicians. She emphasizes the need to begin the transition process early and the importance of patients, caregivers, clinicians and agencies to work together to address barriers and to assure effective transition and transfer of care.
