“Med error podcast: re: “reconciliation.” Sean discussed reconciliation as though it’s related to reconstitution, which it is not. Reconciliation describes looking at the meds the patient is taking at home and/or reviewing meds when at patient is transferred to another unit or is being discharged. IV fluids podcast: Content was more appropriate for a medical audience as the focus was on selecting the fluid, which is not a nursing function. (Would be something a nurse practitioner would be able to do.) He should have focused on the nursing actions related to IVF-calculating fluid requirements to know that the infusion rate is correct and monitoring the insertion site.”
