“I am a seasoned nurse who recently changed from working at a Level 3 obstetrical unit with a wonderful NICU down the hall, in a metropolitan facility for 19+ years, with extensive resources, to a Level 1, rural facility with severely restricted resources- yet I find myself caring for such a wide range of high risk patients who present to us due to the 2+ hour travel requirement to go to a higher level of care, plus their premature and/or sick newborns (until a transport team can arrive). Such a huge change in responsibilities, staff and ancillary services- I have had to completely change my nursing approach- there are no in-house doctors or residents to call on immediately to perform initial assessments for anything outside of normal labor. Listening to your podcasts have helped me to consider how to adapt my practices. You have also inspired me to work with my manager to develop check-lists to improve high reliability performance around common and uncommon OB emergencies. Thank you so much.”
