Awesome study of human factors

I am left wondering if the drain tube in Oxygen tank 2 of Apollo 13, alone should have been clause to scrap that tank before the mission ever left the ground, because I wonder if it had the potential to clog or block the valve. And I hope the upcoming series involves my favorite Polar explorer.
